Summary:The research is intended to observe the performance of the Industrial and Public Places Accreditation Services in the Public Health Office of Kulon Progo Regency, Daerah Istimewa Yogyakarta Province. The method used in this research is qualitative descriptive analysis, done by summarizing or reducing data, serving data, and drawing conclusion. The technique of data collection is taken through observations, interviews, and documentations. According to the result of the research, it is concluded that the perfor-mance of the Public Health Office with the case study of the Industrial and Public Places Accreditation in the Public Health Office of Kulon Progo Re-gency, Daerah Istimewa Yogyakarta Province, has not yet achieved its opti-mum level, observed by its responsiveness, effectiveness, and accountability indicators. The factors affecting the less-than-optimum level of performance of the Public Health Office with the case study of Industrial and Public Places Accreditation in this research is connected to its structure of organization, human resources, and organizational culture. In order to enhance the organizational performance, some steps are to be taken. First, assessing the time needed to obtain a legal permission by simplifying the existing system and procedures
