總結:The purpose of this study was to explore a creating the functional competency of nurses working at an orthopedic unit, tertiary hospital. The Ethnographic Delphi Futures Research (EDFR) Technique was used in this study. Participants were 20 orthopedic experts, including 3 physicians, 4 nursing administrators, 4 nursing instructors, and 3 advance practice nurses in tertiary hospital. The EDFR Technique consisted of 3 steps. Step 1: Experts were interviewed about creating the functional competency of nurses working at an orthopedic unit, tertiary hospital. Step 2: Data from step 1 were analyzed using content analysis. Then, the rating scale questionnaire on sub-elements in each item were created. All items in the questionnaire were rated by all experts. Step 3: Data were analyzed using median and interquartile range. Then, the questionnaire with the median and interquartile range of each item was sent to the same experts for their confirmation. Data were analyzed again using median and interquartile range to summarize a creating the functional competency of nurses working at an orthopedic unit, tertiary hospital.  The functional competency of nurses working at an orthopedic unit, tertiary hospital were classified in to 6 components: 1) Orthopedic nursing, 2) Providing information and counseling, 3) Using orthopedic equipment and appliance, 4) Risk management 5) Communication and using information technology and 6) Quality improvement. The four levels of nurses working at an orthopedic unit have  same major competency but different details as follows: Practitioner Level: Nurses can provide orthopedic nursing and other basic nursing procedures. Professional Level: Nurses can apply in-depth knowledge to care for  complexed signs and symptoms  of critical orthopedic patients and work effectively with multidisciplinary team. Senior Professional Level: Nurses can solve any urgent problem immediately and efficiently, supervise nursing staff and improve nursing quality with empirical evidences. Expert Level: Nurses can  be a consultant of orthopedic nursing care,  promote and support  nursing staff  to develop and disseminate innovations of orthopedic nursing care.
