總結:Endometriosis is one of gynecologic condition characterized by the implantation of endometrial like tissue, where the stromal and gland abnormally grow outside uterine cavity. The frequency of endometriosis patients with infertility are around 20-50% with primary infertility and 15% with secondary infertility. One of the cause of infertility is folliculogenisis. The growth and development of the follicle needs synergic interaction between estrogen, follicle stimulating hormone (FSH) and luteinizing hormone (LH). Variations or CYP19 gene polymorph isms cause changes in expression activity of cytochrome P450 enzyme aromatase, which can lead in estradiol production. The aims of this study are to describe ihe disturbance of folliculogenesis phase in endometriosis patient, the contribution ofCYP 19 gene polymorphism that correlated with SMAD3, FSHr and Cytochrome P450 aromatase expression in patient that following in vitro fertilization program compare to these of non endometriosis. The correlation between polymorphism pattern, the severity of the disease and the success rate of folliculogenesis will be studied. Design of this study is a case control study that analyze CYP19 gene which code cytochrome P450 aromatase enzyme correlate with SMAD3 and FSHr expression and success rate of folliculogenesis. Polymorphism variations are analyzed by Gene Scanning. SMAD3, FSHr and cytochrome P450 aromatase expression by immunocytochemistry. The subject of this study consists of 25 individualis as a control group of endometriosis patient and 25 as a non endometriosis patient group. The Gene Scannings showed that result the length of fragments are 152-191in accordance to (TTTA) in repeat 2-12. In the endometriosis group there are 7 polymorphism groups (1) 6/6,6/6,6/8.6/11, 2/11, 7/12 and 11/12. most cases are 6/1 I genotype or (TTTA)6(TTTA)11 (8 patients, 32%) and the most control groups are 6/6 genotype or (TTTA)6(TTTA)6 (8 patients, 32%). In control group, the dominant homozygote genotypes are 6/6 and 7.317.3is 32 and 12%
